How to submit a ticket to provision a MAC address

When submitting a ticket to provision a MAC address or move a computer to a different department, please provide the following information:

 

For new provisioning:

  • Hostname
  • MAC address
  • User name
  • Department
  • Room number
  • Wall port number

 

If replacing a computer, provide the following for both the new and old computers:

  • Hostname
  • MAC address
  • User name
  • Department
  • Room number
  • Wall port number

 

For WiFi:

  • Hostname
  • MAC address
  • User name
  • Department
  • Network SSID (e.g., OC_LP, OC_AC, OC_CT)

 

MAC Address format for all requests: 012345abcdef (no colons, spaces, or dashes)
